NFL Week 4 Injury Report: Jadarian Price to IR, Keenan Allen Out, Terry McLaurin Update

As the 2026 NFL season hits its first full October weekend, the injury landscape has already reshaped the fantasy football and real-life fortunes of countless teams. With Week 4 upon us, the Vulius NFL Analysis Desk brings you the latest updates from Saturday, October 3, 2026, on the players who matter most. From season-ending injuries to game-time decisions, here’s everything you need to know before setting your lineups and placing your bets.

AFC East: Maye’s Shoulder a Concern; Achane Lost for Season

In New England, rookie quarterback Drake Maye is practicing fully despite a new injury to his throwing shoulder, but he remains a shaky QB2 against a stout Bills defense. The Patriots also placed wide receiver A.J. Brown on injured reserve with a high-ankle sprain, sidelining him for at least four games. Mack Hollins and DeMario Douglas are expected to see increased snaps, while Kyle Williams could emerge as a deep threat. Meanwhile, the Dolphins received devastating news: running back De’Von Achane tore his ACL in Week 3 and is out for the year. Ollie Gordon and Jaylen Wright will split backfield duties, but with Miami’s offense struggling, neither is a reliable fantasy starter. Jaylen Wright (foot) will play in Week 4, offering explosive upside but a tough matchup against the Vikings. For the Jets, Breece Hall (quad) is out, making Braelon Allen a top waiver add. Rookie tight end Kenyon Sadiq (back) is questionable but would be a TE1 if active, especially with Mason Taylor (thumb) also sidelined.

AFC North: Jackson Good to Go; Andrews a TE2

Lamar Jackson (back) was removed from the injury report and will start against the Titans, keeping him as a locked-in QB1. Zay Flowers (hamstring) is questionable but expected to play after the Ravens managed his workload well. Mark Andrews (hand) is a full participant and settles in as a TE2. The Bengals lost Andrei Iosivas (thumb) to IR, opening a competition for the No. 3 receiver role between Colbie Young and Dohnte Meyers. In Cleveland, Dylan Sampson (knee) is on IR, and the Browns’ backfield remains a fantasy wasteland with Deshaun Watson under center. Pittsburgh’s Rico Dowdle (toe) is out, cementing Jaylen Warren as a volume-based RB1.

AFC South: Collins Returns; Allen Out in London

Texans wide receiver Nico Collins (hamstring) will make his return against the Cowboys after a two-game absence, though his snaps may be monitored. Tank Dell (knee) remains on IR, and Jayden Higgins (ACL) is out for the year, leaving Jaylin Noel and Kayshon Boutte to step up. The Colts will be without Keenan Allen (groin) in London, elevating Josh Downs to WR2 status and Tyler Warren to a mid-range TE1. Alec Pierce (heel) is also on IR. The Jaguars emerged from Week 3 unscathed, while the Titans’ Tony Pollard (ankle) is an RB3 and Tyjae Spears (ankle) is droppable.

AFC West: McConkey Questionable; Price to IR

Chargers wide receiver Ladd McConkey (foot) is questionable after a new injury, and if he sits, Tre’ Harris becomes the top target. Keaton Mitchell (ankle) will play, boosting Omarion Hampton’s outlook. The Broncos’ J.K. Dobbins (hip) and Jaylen Waddle (foot) are full go, but rookie Jonah Coleman (ankle) was a surprise IR stash. In Seattle, Jadarian Price (chest) was ruled out and placed on IR, leaving Emanuel Wilson and George Holani to split carries—a boost for Jaxon Smith-Njigba in a pass-heavy matchup. The Chiefs are healthy, and the Raiders lost Jack Bech (forearm) to IR, giving Tre Tucker more slot snaps.

NFC East: McLaurin Unlikely; Dart Out for Season

Washington’s Terry McLaurin (hamstring) is unlikely to play against the Colts in London, thrusting rookie Antonio Williams into a WR2/FLEX role with elite underlying metrics. Stefon Diggs also gets a bump to WR2. Jayden Daniels (elbow) remains sidelined, so Marcus Mariota is a streaming option. The Giants lost rookie QB Jaxson Dart to a season-ending knee injury, a massive blow to Malik Nabers, Isaiah Likely, and Cam Skattebo. Jameis Winston takes over but offers little fantasy appeal. The Eagles will be without DeVonta Smith (hamstring) and Dallas Goedert (knee), signing Zach Ertz for tight end depth. Dontayvion Wicks and rookie Makai Lemon are forced into larger roles. The Cowboys placed Malik Davis (hip) on IR, leaving Javonte Williams as a workhorse RB1.

NFC North: Jefferson Out; Jacobs on Exempt List

Vikings superstar Justin Jefferson (ankle) is out for Week 4 but hopeful for Week 5, making Jordan Addison a WR2. Aaron Jones gets a boost with Jordan Mason on IR. The Bears will start Tyson Bagent at QB with Caleb Williams (hamstring) still sidelined. The Lions’ Isiah Pacheco (back) remains on IR, and Jahmyr Gibbs continues to dominate touches. Green Bay’s Josh Jacobs is on the commissioner exempt list and could face a 4-6 game suspension, leaving MarShawn Lloyd as the lead back. Jayden Reed (neck) is out indefinitely after surgery, concentrating targets on Christian Watson, Matthew Golden, and Tucker Kraft.

NFC South: Mayfield Out; Etienne Sidelined

Buccaneers quarterback Baker Mayfield (thumb) will miss at least three weeks, a significant downgrade for Emeka Egbuka, Chris Godwin, and Cade Otton. Rookie Jalon Daniels takes over. Bucky Irving (glute) and Chris Godwin (ankle) will play but are risky starts. The Saints’ Travis Etienne (hamstring) will miss time, leaving Alvin Kamara and Kendre Miller in a frustrating committee. The Panthers’ Jalen Coker (quad) is questionable, and if he sits, Tetairoa McMillan becomes a WR1. Bryce Young (knee) is good to go as a low-end QB1. The Falcons are healthy.

NFC West: Nacua Returns; Evans Game-Time Decision

Rams wide receiver Puka Nacua (groin) will play but may face a snap count, keeping Davante Adams as a top WR1. Tight end Terrance Ferguson (ankle) is on IR, making Tyler Higbee a borderline TE1. The 49ers’ Mike Evans (ribs) is a true game-time decision, and if he sits, Christian McCaffrey, George Kittle, and Deebo Samuel will absorb targets. Demarcus Robinson (ankle) is out, giving Jacob Cowing an opportunity. The Seahawks’ Zach Charbonnet (knee) could have his practice window opened, but Jadarian Price (chest) is out, leaving a backfield committee. The Cardinals are healthy.
